How does the Nuchal cord affect my baby? What is a nucal cord? The Nuchal cord is the term used by medical professionals when your baby has her umbilical cord wrapped around her neck. Health professionals check a cord around the neck of each born baby, and it's usually as simple as sliding gently out so it doesn't fit around the baby's neck once the baby has started breathing. If you have a nucal cord diagnosed during pregnancy, there is no more action to take. Your healthcare providers will not suggest the urgent delivery of the baby. Complications Any complication that arises from a nucal cord is extremely rare. It is important to manage your stress levels. The umbilical cord can be compressed during contractions. That reduces the amount of blood pumping to your baby. This can lead to a decrease in your baby's heart rate. Outlook In most cases, a nucal cord is in no way dangerous to the mother or baby. In rare cases where complications occur, your health care team is more than equipped to face them. Babies are usually born safe and well after a nucal cord complication. It is important to remember that the nucal cords cannot be prevented. There's nothing a biological mother does to make it happen. If the baby has been diagnosed with nucal cord, it is best to try not to worry about this condition. The likelihood of asphyxiation and other birth injuries is influenced by the type of nucal cord. Nuchal cords are classified into two main types: Type A Nuchal Cord over "unlocked"Type B Nuchal Cordunder "locked" As mentioned above, a nucal cord may also contain individual or multiple loops; multiple loops may pose a higher risk of fetal complications. Furthermore, the risk of asphyxiation by birth depends on how tight or loose a nucal cord is. If the cord is tighter, the risk of asphyxiation can be higher. It is important to note that the fetal movement can sometimes cause a loose nucal cord initially to tighten (1).tight A nucal cord with fetal hypoxia resulting nasal cords and birth lesions Potential complications and birth lesions resulting from a nucal cord include the following (1):Popoxy-ischemic encephalopathy (HIE)Fetal death Intrauterine growth (IUGR)Meconium in the amniotic fluid: Phertal abnormalities of the heart rate Acid of nullary artery A nucal cord may be more likely to form under the following circumstances (1, 2, 3): As the baby's gestational age increases, the likelihood of a nucal cord also increases. Diagnosis of a nucal cord Nuchal cords are often identified during obstetric ultrasound. To identify a nucal cord, doctors look at multiple views of the fetal neck. A nucal cord is diagnosed when the umbilical cord is seen surrounding at least three quarters of the fetal neck. In term, the ultrasound sensitivity to detect the nucal strings is about 70% for the image in grey scale, and 83% – 97% with Doppler color (1). Signs and symptoms of a nucal cord Pregnant women usually do not experience symptoms of a nucal cord. If the nucal cord is affecting the baby's oxygenation, then the baby can start showing as an abnormal heart rate on the fetal monitor (1). Treatment of a nucal cord The main objective in the management of a nucal cord is to prevent during delivery. Preservation of an intact nucal cord will depend on how tight it is wrapped around the baby's neck. A loose nucal cord can easily slip over the baby's head to decrease the traction during the handover of the shoulders or the body. If this is not possible due to stiffness, there is a technique in which the doctor can be able to slide the cord over the baby's shoulders and deliver the baby through the loop. If this is not possible, the doctor can use the apprentice technique, which allows the shoulders and the body to be born in an apprentice, with the cord that breaks down after the baby is delivered. In rare cases, other interventions, such as one, should be performed (1, 3). A delay in the realization of a necessary caesarean section may severely exacerbate the harmful effects of hypoxia and ischemia and may cause serious brain injuries. The doctor and medical team must be very knowledgeable and skilled in managing problems that arise during delivery and the delivery that can deprive the baby of blood and oxygen. The team must be able to act quickly. Failure to properly monitor and treat the mother and baby during pregnancy, work and childbirth is medical negligence.
